What is home health care?

Home health care provides skilled medical services in the comfort of your own home. These services may include nursing care, therapy, and medical social work to support recovery, manage chronic conditions, and promote independence.

The goal is to help you heal safely, maintain your health, and continue living comfortably at home.

Who qualifies for Home Health Services?

You may qualify for home health care if:
  • You are enrolled in Medicare Part A and/or Part B
  • You are under the care of a licensed healthcare provider (such as a physician, nurse practitioner, physician assistant, or surgeon)
  • You require skilled services, including nursing or physical, occupational, or speech therapy
  • You are considered homebound, meaning leaving home requires considerable effort or assistance
In most cases, Medicare covers 100% of approved home health services for eligible patients.

What insurance do you accept?

We currently accept Original Medicare (Part A and/or Part B).
At this time, we do not accept Medicare Advantage plans, private insurance, or Medicaid.

What are your services?

Do I need a referral to get started?

Yes, home health services require a referral or order from an authorized healthcare provider, such as a physician, nurse practitioner, physician assistant, or surgeon.

Once we receive your referral and verify your Medicare eligibility, our team will contact you to arrange your in-home assessment.

What does ā€œhomeboundā€ mean?

A person is considered homebound when leaving the home requires significant effort, assistance, or is medically inadvisable due to a health condition.

You may still leave home occasionally—for medical appointments, religious services, or short outings—and remain eligible for care.

Will Medicare cover the cost of services?

If you meet Medicare’s eligibility criteria, approved home health services are typically covered at 100%.

There are no out-of-pocket costs for covered services, though any non-covered services may incur separate charges.

How often will someone visit my home?

The frequency of visits depends on your individual condition and the care plan established by your healthcare provider.

Our team will coordinate a schedule that supports your recovery and ongoing health needs.

Do you provide 24-hour or live-in care?

No, home health care is provided on a part-time, intermittent basis and is intended for short-term medical needs. Medicare does not cover continuous or live-in care services.

Can my family or caregiver be involved?

Yes, we encourage family and caregiver participation. Our team can provide education and guidance to help support your care plan and promote independence at home.
